Object To explore the clinical efficacy of a narrow‐band intense pulsed light (DPL) with different energy density in the treatment of erythematotelangiectatic rosacea (ETR). Methods A retrospective study was conducted on patients diagnosed with erythema telangiectatic rosacea who received 5 DPL treatment sessions in the laser cosmetology department of a hospital from October 2021 to October 2023. The patients were divided into two groups according to the mean energy density of DPL treatment: low‐energy group (5.0–6.0 J, 12 ms) and high‐energy group (6.1–7.1 J, 12 ms). The red value, Clinician's Erythema Assessment (CEA) score, and recurrence rate were analyzed to evaluate the efficacy of the treatment. Adverse reactions were also evaluated. Results A total of 133 patients were enrolled in the study. After treatments, the red value was significantly decreased (16.03 ± 6.71 vs. 13.41 ± 4.35, respectively, p < 0.05), and CEA score was also significantly decreased (2.41 ± 0.97 vs. 1.71 ± 0.93, respectively, p < 0.05). A total of 69 patients received low energy therapy (5.0–6.0 J, 12 ms) and 64 patients received high energy therapy (6.1–7.1 J, 12 ms). The improvement of red value in low‐energy group (3.79 ± 5.19) was significantly better than that in high‐energy group (1.37 ± 5.09), p < 0.05; For CEA score improvement, the low‐energy group was also better than the high‐energy group (0.83 ± 1.08 vs. 0.55 ± 1.19, respectively, p > 0.05). One year after the treatment, the recurrence rate in the low‐energy group was significantly lower than that in the high‐energy group (9.2% vs. 22.4%, p = 0.043, OR = 2.841, 95% CI: 1.002–8.055). Three patients developed postoperative allergic reactions. Conclusions DPL can significantly improve erythema and telangiectasia in patients with ETR, and treatment with mild energy density has a better effect.
