Cross-sectional study reports surgical outcomes and high satisfaction in transgender women after modified PIV surgery.
Introduction The modified penile inversion vaginoplasty (PIV) represents an effective surgical approach for male-to-female (MtF) gender-affirming surgery. Objective This study aimed to present surgical results, long-term follow-up data, and satisfaction outcomes of transgender women who underwent the modified PIV technique at a single institution in Turkey, which to our knowledge constitutes the largest series reported from the country. Methods A cross-sectional study was conducted including adult transgender women who underwent gender-affirming surgery with the modified PIV technique between January 2017 and December 2024. Demographic characteristics, pre- and postoperative clinical data, and complication rates were prospectively recorded. Long-term complaints and overall satisfaction were assessed through virtual follow-up interviews. Results Forty-five patients were included, with a mean age of 32.57 ± 7.08 years. The mean postoperative hospital stay was 9.75 ± 1.39 days, and the mean vaginal depth at the one-year follow-up was 14.7 ± 2.16 cm. Adequate neovaginal lubrication was reported by 38 (84.44%) participants. No complications occurred in 16 (35.55%) cases, while intraoperative (rectal injury) and postoperative (meatal stenosis, vaginal narrowing, scarring, infections, etc.) complications were observed in 29 (64.44%) patients. The overall satisfaction rate, including psychological, physical, and social domains, was 83.44% (range: 66–97%). Conclusions The modified PIV technique provides satisfactory aesthetic and functional results, achieving sufficient vaginal width, depth, and lubrication. It appears to be a particularly effective option for patients with limited penile skin availability. Disclosure No
