Introduction and Objective: Nearly 3 in 10 U.S. women have pre-pregnancy overweight/obesity associated with an increased likelihood to develop diabetes. Lifestyle modifications and breastfeeding longevity may reverse this effect. The purpose of this study was to quantify interest in use of the eMOMSTM mobile health application and to measure weight retention and breastfeeding duration among women with pre-pregnancy BMI ≥25. Methods: A feasibility, two-arm, randomized controlled trial was completed and included a 6-month intervention modelled after the national Diabetes Prevention Program and the Office on Women’s Health “Your Guide to Breastfeeding.” It was delivered using the eMOMSTM app and phone follow-up by a health coach. Continuous outcomes were analyzed with independent samples t-test, linear regression and/or linear mixed-effects models adjusting for baseline value and prespecified covariates. SAS (Version 9.4; SAS/STAT Institute, Inc., Cary, NC, USA) was used for all analyses. Results: Between May 1-December 31, 2024, 125 individuals were screened, of which 43 were randomized: 22 to intervention (I) and 21 to control (C). Study completion rate was 86.1%. At baseline, pre-pregnancy BMI was 30.4 ± 3.4 (I) vs. 30.3 ± 3.1 (C), p=0.97; age was 31.7 ± 6.5 (I) vs. 28.1 ± 5.5 (C); 13.6% (I) vs. 47.6% (C) of participants were rural; 28.6% (I) vs. 33.3% (C) were Hispanic; 45.5% (I) vs. 23.8% (C) participated in WIC; 50% (I) vs. 42.9% (C) reported being a first-time mother; and 45.5% (I) vs. 23.8% (C) reported having a biological family member treated for diabetes. No statistically significant differences were observed in the main outcome measures: intervention participants had reduced postpartum weight retention at 3.18 lbs. (16.01) vs. control at 11.01 lbs. (17.74), p=0.18; intervention participants exclusively breastfed for 6.8 weeks (5.89) vs. control at 8.0 weeks (5.51), p=0.55. Conclusion: Lifestyle changes coupled with breastfeeding duration appear to impact women’s health positively and further research in this area is warranted. Disclosure L.T. Jacobson: None. H.
