Prospective cohort study evaluates muscle loss incidence in prediabetics, suggesting lifestyle changes may help preserve muscle mass.
Introduction and Objective: Severe muscle loss (SML) is a progressive condition often associated with aging and metabolic dysfunction. While a bidirectional link exists between diabetes and muscle depletion, data regarding the incidence and predictors of SML in individuals with prediabetes remain limited. This prospective cohort study aimed to evaluate SML incidence and identify its predictive factors within people with prediabetes. Methods: This study included 199 high-risk participants with prediabetes (age 59.9 years; BMI 25.9 kg/m²). Body composition and function was assessed using bioelectrical impedance analysis and Handgrip Strength. Participants were followed for 3 to 5 years, with SML defined by age-stratified muscle loss rates (either loss ≥3% or ≥1%/year in age ≥75 or <75 year, respectively). Multivariate logistic regression was utilized to determine independent predictors of SML. Results: Among the 199 individuals with prediabetes (age 59.9 years, BMI 25.9 kg/m2), the incidence of SML was 26.6% (n=53), while 5.29% (n=10) of participants were diagnosed with sarcopenia by the end of the follow-up. Significant predictors of SML included advanced age (OR1.06,1.01-1.11; p=0.02) and prolonged daily sitting duration (1.22,1.08-1.37;p=0.002). Conversely, higher meat consumption (0.48, 0.23-0.99;p=0.04), statin use (0.32; 95% CI: 0.16-0.67;p=0.003), and higher AST levels (0.94, 0.89-0.99;p=0.04) demonstrated significant protective effects. Conclusion: SML is prevalent among individuals with prediabetes. Advanced age and sedentary behavior are primary risk factors, whereas higher protein intake and statin therapy may mitigate muscle decline. These findings emphasize the importance of lifestyle and pharmacological interventions in preserving muscle mass during the prediabetes stage. Disclosure C. Washirasaksiri: None. W.
