Cross-sectional analysis shows that meeting multiple metabolic targets lowers complication rates in type 2 diabetes, suggesting a shift in treatment focus.
Introduction and Objective: Current diabetes management focuses predominantly on glycemic control, yet emerging evidence suggests metabolic parameters may drive complications more strongly than HbA1c alone. We investigated whether simultaneous achievement of multiple metabolic targets (triglycerides <150 mg/dL, BMI 23-25, HbA1c <7%) provides synergistic protection against multi-system complications compared to achieving individual targets in community-based type 2 diabetes Methods: Cross-sectional analysis of 400 T2DM patients from community screening camps with comprehensive assessment including HbA1c, lipid profile, BMI, bone mineral density, and peripheral neuropathy. Patients stratified by number of targets achieved: Group A (0-1 target), Group B (2 targets), Group C (all 3 targets). Primary outcome: multi-system complications (≥2 of bone disease, neuropathy, dyslipidemia). Results: Among 400 patients, only 18% achieved all three targets versus 45% achieving 0-1 target and 37% achieving 2 targets. Multi-complication rates showed dose-response: Group A (0-1 target) 68%, Group B (2 targets) 42%, Group C (all 3 targets) Triple-target achievement provided 73% relative risk reduction versus single-target (RR 0.26, 95% CI 0.14-0.49). Critically, patients achieving TG+BMI targets without optimal HbA1c (n=52, HbA1c 7.2-8.1%) had significantly lower complications than those achieving HbA1c<7% without TG+BMI targets (n=48): 28% versus 54% (p<0.01). Specific complication comparison across groups: severe bone loss (osteoporosis) 8% vs 32% vs 61% (p<0.001), moderate-severe neuropathy 12% vs 38% vs 58% (p<0.001) Conclusion: Simultaneous achievement of triglyceride, BMI, and HbA1c targets provides synergistic 73% complication reduction beyond individual target achievement. Metabolic targets (TG+BMI) demonstrate greater protective value than glycemic control alone. Findings support paradigm shift from glucose-centric to comprehensive tri-parameter management with concrete targets: TG<150, BMI 23-25, HbA1c<7%. Disclosure S.D. Sonawala: None.
