Evaluates hypoglycemia and hospital stay duration in diabetic patients, indicating significant improvement with specialized program.
Introduction and Objective: Hypoglycemia remains a frequent complication during hospitalization in subjects with diabetes. Some reports indicate that proactive diabetes management by a specialized diabetes team (SDT) reduces the length of stay (LOS), but its specific impact on hypoglycemia has not been explored. The main aim of the present study is to evaluate whether the management of diabetes by a SDT exerts any beneficial effect in terms of reducing the rate of hypoglycemia and LOS. Methods: A Smart Diabetes Hospital (SDH) program consisting of a glycemic map linked to SDT (endocrinologist + diabetes nurse) has been implemented progressively in medical and surgical floors at Vall d’Hebron University Hospital. Data regarding glycemic control, hypoglycemia, clinical characteristics and LOS were collected. A total of 406 vs. 494 subjects before and after implementation of SDH were compared. Results: A total of 900 subjects were included (45.1% before and 54.9% after SDH program). As a whole, 9.7% presented hypoglycemia during the hospitalization. Patients with hypoglycemia present larger LOS (26.1±25 vs 13.9±13, p<0.001) and higher mortality at 3 months of follow up (26.5% vs 9.5%, p=0.002). The SDH program was able to reduce the number of patients with hypoglycemia (13.3% vs 6.7%, p<0.001). Furthermore, the LOS (16.49±17.4 vs 14.14±13.2, p<0.001) and mortality at 3 months (5.3% vs 2.8%, p=0.021) were also reduced. Conclusion: a) Hypoglycemia is an essential risk factor for extending the LOS and is associated with higher mortality rate after discharge. b) The implementation of SDH program reduces hypoglycemia and LOS in hospitalized subjects.
