1723-P: Semaglutide Reduces Asthma-Related Adverse Outcomes in Patients with Comorbid Asthma and Obesity: A Post Hoc Analysis of the SELECT Trial

Introduction and Objective: Obesity is a common comorbidity with asthma. In a post hoc analysis, we investigated the effect of once-weekly semaglutide 2.4 mg vs placebo on asthma-related adverse/serious adverse events (AEs/SAEs) in patients (pts) with asthma and overweight or obesity. Methods: SELECT was a randomized, double-blind, multicenter, placebo-controlled, phase 3 trial in adults ≥45 years, with a BMI ≥27 kg/m2 and established cardiovascular disease. In this analysis, pts with self-reported asthma were followed for asthma-related AEs/SAEs. Changes in biomarkers (blood high-sensitivity C-reactive protein [hsCRP], eosinophil and neutrophil counts; and Olink Flex panel) from baseline to week 104 were evaluated. Results: A total of 1190 pts reported asthma (median age 62 years; median BMI 33 kg/m2; 44% female) with similar baseline characteristics in pts treated with semaglutide (n=588) vs placebo (n=602). Mean body weight reduction was greater with semaglutide (9.7%) vs placebo (0.9%), and semaglutide reduced hsCRP by 38.9%, but with no changes in blood eosinophil or neutrophil counts. Overall, pts treated with semaglutide had lower incidence of asthma-related AEs/SAEs (n=27) vs placebo (n=46; hazard ratio 0.58; 95% CI 0.36, 0.93; Fig). Conclusion: Semaglutide reduced asthma-related AEs/SAEs vs placebo, supporting further investigation of semaglutide in comorbid asthma and obesity. Disclosure C.S.
