Introduction and Objective: It is important to understand if the effect of semaglutide 2.4mg (sema) on major adverse cardiovascular events (MACE) varies by race/ethnicity given prevalent disparities. This SELECT trial post hoc analysis assessed the efficacy and safety of sema for MACE risk reduction in patients with overweight/obesity and CVD without diabetes stratified by race/ethnicity. Methods: Participants randomized to receive sema or placebo were grouped by self-reported race/ethnicity to assess the treatment effect on MACE. Cox regression models testing for interactions (race/ethnicity x treatment effect) were performed; adverse events (AEs) leading to discontinuation were summarized descriptively. Results: Race/ethnicity were balanced across treatment arms (Figure). Sema-treated patients exhibited similar reductions in weight from baseline (Asian, Other, White, Hispanic, not Hispanic: -9%; Black: -10%). Sema consistently reduced the risk of MACE across race/ethnicity. Asian patients receiving sema had slightly greater MACE risk reduction vs those receiving placebo. The proportions of sema-treated patients with AEs (Asian: 11%; Hispanic: 15%; Black: 16%; Other, White, not Hispanic: 17%) and serious AEs (Asian, Hispanic: 3%; White, not Hispanic, Other: 4%; Black: 5%) leading to discontinuation were similar across races/ethnicities. Conclusion: Sema reduced the relative risk of MACE and had a consistent safety profile regardless of patient race or ethnicity. Disclosure J.
