Digital DiaProfil screening reveals ~29% of diabetic adults lack self-management confidence and report poor health.
Why the study?
To implement and evaluate a seamless digitalized national patient-reported outcome diabetes tool to automatically collect and present data, enabling person-centered diabetes care across hospitals in the North Denmark Region.
Cross-Sectional (n=6,676)
Yes
Implementation of a digital PROMs tool in routine diabetes care successfully captured detailed bio-psycho-social profiles, revealing significant proportions of patients with poor perceived health and worries about complications.
Signals substantial self-management gaps in diabetes; hypothesis-generating for routine digital PROMs.
Introduction and Objective: Our VBHC-PROMS research team has developed and fully implemented a one-click seamless digitalized national Danish digital Patient-Reported Outcome (PROMS) diabetes tool, automatically collecting and presenting data, thereby enabling person-centered diabetes care at all four hospitals in the North Denmark Region. Methods: Prior to attending the diabetes outpatient clinic, the person with diabetes fills in questionnaire, consisting of 92 items, hereoff 20 demographic and 72 bio-psycho-social describing 9 domains of interest: 1) Perceived Health, 2) Mental Well-Being, 3) Symptoms, 4) Daily Life with Diabetes, 5) Worries about Complications, 6) Confidence in Self-Management, 7) Blood Sugar Control, 8) Medicine Experience and 9) Access to Diabetes Specialists. Data are instantly transferred to the Clinicians Dashboard and are automatically logged to national databases and for hospital management use. Results: 6676 persons with diabetes (adults, 18-99 years of age, T1DM and T2DM) delivered 480672 scientifically validated and detailed bio-psycho-social PROMS parameters, describing individual scores of the 9 domains described above. Response rate 70%. Preliminary results: 21.6% poor self-perceived health, 24.9% constant worries about complications, 28,7% not confident in self-management of diabetes. Mental well-being was not clearly associated with HbA1c. A full data report on all 9 domains will be presented associated to diabetes type, age groups, gender and full clinical characterizations. Conclusion: Our national psychometric PROMS Questionnaire & Decision-Algorithm built on nine detected bio-psycho-social domains accurately identifies and describes needs and wishes of persons with diabetes.. This is the first real-time PROMS data collection to be used fully implemented in routine diabetes care, connecting the person with diabetes, the healthcare provider and the national IT-infrastructure. NIELS EJSKJAER (2026) conducted a cross-sectional in Diabetes (T1DM and T2DM) (n=6,676). Digital DiaProfil Questionnaire was evaluated on Bio-psycho-social PROMS parameters across 9 domains. Implementation of the digital DiaProfil PROMs questionnaire in 6,676 adults with diabetes revealed 21.6% had poor self-perceived health and 28.7% lacked confidence in self-management.
