Explores social factors affecting quality of life in diabetic foot ulcer patients, suggesting intervention opportunities.
Introduction and Objective: Little is known about social drivers of diabetic foot disease (SDDFD) and which specific drivers influence quality of life (QOL) in individuals with diabetic foot ulcers (DFU). We hypothesized that social vulnerabilities would be associated with worse QOL in patients with active DFU. Methods: This study used baseline assessments from the Diabetic Foot Consortium multicenter prospective cohort of open DFU patients. SDDFD were assessed using the SDDFD battery capturing 10 binary domains: food security, health literacy, housing security, English proficiency, social support, education, income, government assistance, insurance, and self-care ability. QOL was assessed using the Diabetic Foot Scale Short Form (DFS-SF) total score (0-100; higher is better). Linear regressions were performed for each SDDFD domain individually and in a model with all SDDFD domains. Results: Of 667 participants with active DFU, 523 (77%) had complete responses for all SDDFD domains (20% missing income). Mean (SD) participant age was 59 (12) years, 78% were male, 61% White, 17% Black, 18% Hispanic, and 92% type 2 diabetes. Food insecurity, limited health literacy, and difficulty with self-care significantly decreased DFS-SF on adjusted analysis (Table 1). Conclusion: Food insecurity, limited health literacy and difficulty with self-care are associated with worse QOL in patients with DFU. These factors represent opportunities for directed interventions to improve QOL in patients with active DFU. Disclosure J. Iannuzzi: None. M. Brennan: None. A. Oropallo: None. W. Ye: None. C.
