Systematic review demonstrates GLP-1 receptor agonists improve cardiometabolic outcomes in schizophrenia without worsening psychosis.
Introduction and Objective: Antipsychotics can cause metabolic dysfunction in people with schizophrenia spectrum disorders. We assessed whether GLP-1 receptor agonists (GLP-1 RAs) improve cardiometabolic outcomes without worsening psychiatric symptoms. Methods: We searched MEDLINE, Embase, PsycINFO, Cochrane Central, and ClinicalTrials.gov for randomized trials comparing GLP-1 RAs with placebo or usual care in adults with schizophrenia spectrum disorders receiving antipsychotics and presenting with prediabetes, obesity, overweight or impaired glucose metabolism .Five RCTs (n=355) were included. We extracted baseline-to-end-of-treatment changes in cardiometabolic outcomes (glycemia, weight-related measures, blood pressure, lipids) (Fig. 1). We pooled mean differences (95% CI) using random-effects with restricted maximum likelihood estimation and Hartung-Knapp adjustment. Results: Adjunctive GLP-1 RA therapy significantly reduced body weight (MD −8.02 kg), BMI (MD −2.56 kg/m²), and HbA1c (MD −0.32%); other pooled outcomes are shown in Fig. 1. Psychosis ratings remained stable. Gastrointestinal adverse events were more common without higher discontinuation. Conclusion: In adults with schizophrenia spectrum disorders receiving antipsychotics, GLP-1 RAs improved cardiometabolic outcomes without worsening psychosis. Disclosure J. Srivastava: None. V.
