18F-FDG reveals intense uptake while 68Ga-DOTA-FAPI-04 shows minimal accumulation in a cervical chordoma case.
Why the study?
Does 18F-FDG PET/CT show different uptake patterns compared to 68Ga-DOTA-FAPI-04 in cervical chordoma?

Does 18F-FDG PET/CT show different uptake patterns compared to 68Ga-DOTA-FAPI-04 in cervical chordoma?
18F-FDG PET/CT may show more intense uptake than 68Ga-DOTA-FAPI-04 in cervical chordoma, highlighting potential heterogeneity in FAPI-targeted imaging for this tumor type.
Discordant intense FDG but minimal FAPI uptake in chordoma cautions against assuming uniform tracer avidity; leaves open FAPI PET reliability in this rare tumor.
Chordoma is a rare malignant bone tumor derived from notochordal remnants, typically occurring in the axial skeleton. Herein, we report a case of an 81-year-old woman with a cervical chordoma involving the C1-C5 vertebrae. The lesion demonstrated intense 18F-FDG uptake but minimal 68Ga-DOTA-FAPI-04 accumulation. This discordant imaging pattern highlights that while FAPI-targeted PET/CT is a promising pan-cancer imaging modality, its uptake in chordoma may be heterogeneous, potentially influenced by the tumor microenvironment or specific histologic subtypes.

Xu et al. (2026) conducted a case report in Cervical chordoma (n=1). 18F-FDG PET/CT and 68Ga-DOTA-FAPI-04 PET/CT was evaluated on Imaging uptake pattern. In an 81-year-old woman with cervical chordoma, the lesion demonstrated intense 18F-FDG uptake but minimal 68Ga-DOTA-FAPI-04 accumulation, highlighting heterogeneous FAPI uptake in chordoma.
