Surgical aortic valve replacement using a rapid-deployment INTUITY valve is a viable bailout option for patients with a failed SAPIEN 3 transcatheter valve when redo TAVI is contraindicated.
Objective: We describe the first reported successful case of rapid-deployment aortic valve implantation using the INTUITY valve (Edwards Lifesciences, Irvine, CA, USA) performed after failure of a transcatheter aortic valve implantation with a SAPIEN 3 valve (Edwards Lifesciences). Case Presentation: A 61-year-old woman with severe asbestosis and systemic scleroderma-related respiratory dysfunction presented with severe symptomatic aortic stenosis due to structural deterioration of a 23-mm SAPIEN 3 device implanted 8 years earlier. Preoperative computed tomography raised concern about right coronary ostium obstruction during redo valve deployment; therefore, transcatheter valve-in-valve therapy was abandoned. Surgical explantation was undertaken as bailout surgery. To reduce the aortic clamp time, the INTUITY valve was chosen. A 19-mm INTUITY valve was deployed after explantation of the SAPIEN 3 valve and annular decalcification. Transthoracic echocardiogram demonstrated an aortic valve gradient of 27/13 mmHg with mild residual aortic regurgitation. The patient was discharged in a stable condition with favorable hemodynamic performance 20 days postoperatively. Conclusion: This case demonstrates that INTUITY valve implantation can represent a viable surgical option for patients with a failed SAPIEN 3 valve when repeat transcatheter intervention is contraindicated.
