Radiofrequency ablation can be used to treat PVCs originating from the aortomitral continuity in patients with structural heart disease such as prior mitral valve replacement.
The knowledge about ventricular arrhythmias arising from the aortomitral continuity (AMC) location is still limited. In this case we report the ablation of premature ventricular contractions (PVCs) arising from this particular region in a patient with a recent history of mitral valve replacement surgery recently. A 53-year-old female patient with a history of mitral valve replacement 6 months ago was referred for frequent PVCs on 24 h Holter monitoring. We have performed the ablation with radiofrequency delivery of PVCs that originated from the AMC region. Although ventricular arrhythmias arising from was occurred in structurally normal hearts in general, they may be seen in patients with structural heart disease. As our patients, mitral valve surgeries may result in fibrosis in the peri-AMC region and consequently scar-related reentry, which triggers ventricular arrhythmias.
