Prospective multicenter study reveals lifestyle and tumor characteristics influence survival in non-cirrhotic HCC.
The epidemiological landscape of hepatocellular carcinoma (HCC) is shifting from viral-dominated etiologies toward metabolic dysfunction-associated steatotic liver disease (MASLD). While the majority of cases still occur within the context of cirrhosis, approximately 20% of HCCs now arise in non-cirrhotic livers (NC-HCC). The prospective multicenter Spanish study by Romero-Gutiérrez et al. provides a timely analysis of this cohort, revealing that while surgical outcomes are favorable, long-term survival is heavily influenced by two factors: pathological aggression and patient lifestyle.
