A BSTRACT Germ cell tumours account for approximately 30% of all ovarian neoplasms and 6% of all cancers in women. The most common type of tumour is a benign cystic teratoma, which accounts for approximately 95%. The proportion of cases in which malignancy occurs is 1–2%. Squamous cell carcinomas account for 75–90% of all cancers. A 58-year-old postmenopausal woman, presented with the complaint of pain in right lower abdomen for 1 month and a large complex abdomino-pelvic mass on examination and investigations. The final histopathology was reported as squamous cell carcinoma of the right ovary arising from a mature cystic teratoma. This case highlights the rare occurrence of a mature cystic teratoma transforming into squamous cell carcinoma, particularly in postmenopausal women. Early detection and immediate surgical treatment are crucial to prevent a poor prognosis, given the aggressive nature of the cancer.
