Key result
TCAR is linked to ~28% fewer postoperative neurological events than CEA in symptomatic patients ≥80 years.
Observational (n=358,282)
Yes
Effect estimate: OR 0.72 (95% CI 0.58-0.88)
p-value: p=0.002
Does not support changing practice to favor TCAR in symptomatic patients ≥80; leaves open confirmation in randomized trials.

Zil-E-Ali et al. (2026) conducted an observational in Carotid disease (n=358,282). Transcarotid artery revascularization (TCAR) vs. Carotid endarterectomy (CEA) was evaluated on Postoperative neurological events (stroke/TIA) in symptomatic patients aged ≥80 years (OR 0.72, 95% CI 0.58-0.88, p=0.002). In symptomatic patients aged ≥80 years, transcarotid artery revascularization was associated with significantly lower odds of postoperative neurological events compared with carotid endarterectomy (OR 0.72).
