Aims Alcohol and tobacco remain leading contributors to preventable disease worldwide, and their concurrent use is associated with compounded health risks. This review synthesizes quantitative evidence examining the reciprocal relationship between alcohol and tobacco use in adults, with emphasis on how each behavior affects the initiation, continuation, and relapse of the other.Method A structured search was conducted across PubMed, Wiley Online Library, ScienceDirect, and Google Scholar using combinations of keywords including “alcohol use,” “tobacco use,” “co-use,” and “relapse.” Quantitative studies involving adults that evaluated both alcohol-to-tobacco and tobacco-to-alcohol pathways were included.Results Twelve studies met the inclusion criteria. Nine studies reported alcohol-to-tobacco effects, eight reported tobacco-to-alcohol effects, and six demonstrated bidirectional associations. Alcohol use was consistently linked with increased smoking behavior and relapse risk, whereas the impact of tobacco use on alcohol outcomes showed greater variability across study designs.Conclusion The interdependent nature of alcohol and tobacco use presents challenges when these behaviors are addressed separately. Interventions that simultaneously target both substances are likely to yield better outcomes and reduce the overall burden associated with their co-use.
