Balloon nudge technique enables coaxial alignment and native valve crossing during challenging horizontal-aorta TAVR.
Why the study?
Anatomic factors and device limitations may increase technical difficulty during transfemoral transcatheter aortic valve replacement for severe aortic stenosis.

The novel balloon nudge technique offers an effective, low-risk solution for achieving coaxial alignment to cross difficult native valves during TAVR, circumventing the need for rapid pacing or predilation.
Transfemoral transcatheter aortic valve replacement is an effective treatment for severe aortic stenosis with a high rate of procedural success with the current devices. However, anatomic factors and device limitations may increase technical difficulty. We describe the balloon nudge technique, a novel technique that improves coaxial alignment while crossing the aortic valve. (Level of Difficulty: Advanced.)

Lucarelli et al. (2022) conducted a case report in Severe aortic stenosis (n=1). Balloon nudge technique was evaluated on Procedural success (successful native valve crossing and prosthesis deployment). The balloon nudge technique successfully enabled coaxial alignment and crossing of the native aortic valve during TAVR in a patient with an unfavorable horizontal aortic angle.
