Key result
Anterior mitral leaflet extension plus myectomy is linked to less postoperative MR and SAM than myectomy alone.
Why the study?
Mitral leaflet extension is a novel surgical approach in hypertrophic obstructive cardiomyopathy, but its clinical and functional results when combined with myectomy needed description.
Comparison
Mitral leaflet extension plus myectomy vs myectomy alone
Design
Comparative cohort study

“For the subset of patients with HCM and leaflet elongation but minimal septal hypertrophy, myectomy is often not adequate for complete relief. Our procedure offers added insurance against negative outcomes for these individuals.”
May reduce MR and improve function with leaflet extension plus myectomy in HOCM; hypothesis-generating from small cohort, needs randomized confirmation.
Cohort (n=20)
Kofflard et al. (1996) conducted a cohort in Hypertrophic obstructive cardiomyopathy (n=20). Combined anterior mitral leaflet extension and myectomy vs. Myectomy alone was evaluated on Postoperative clinical and functional parameters (mitral regurgitation, systolic anterior motion, functional class, septal thickness). Combined anterior mitral leaflet extension and myectomy resulted in less postoperative mitral regurgitation (p<0.005) and systolic anterior motion (p<0.001) compared to myectomy alone.
