Subannular Bentall with TAVI explant and homograft successfully repairs root pseudoaneurysm and severe AR.
Case Report (n=1)
This case illustrates the technical challenges and lifetime management strategy required for complex redo aortic interventions in younger patients.
Snare-assisted TAVI explant enables complex reoperation in endocarditis; hypothesis-generating for lifetime management strategies in young aortic patients.
We present the case of a young man with a prior homograft root replacement and a subsequent transcatheter aortic valve-in-valve, who presented with a root pseudoaneurysm and severe aortic regurgitation due to valve degeneration from endocarditis. The surgical procedure involved the explant of transcatheter aortic valve implantation prosthesis using a snare technique, removal of the homograft and a subannular Bentall procedure. This case illustrates the technical challenges and the "lifetime management" strategy required for complex redo aortic interventions in younger patients.

Galeazzi et al. (2026) conducted a case report in Root pseudoaneurysm and severe aortic regurgitation due to valve degeneration from endocarditis (n=1). Explant of TAVI prosthesis using a snare technique, removal of homograft, and subannular Bentall procedure was evaluated. A subannular Bentall procedure with explant of a TAVI prosthesis and homograft was successfully performed in a young man with a root pseudoaneurysm and severe aortic regurgitation.
