ABSTRACT Large peri‐membranous ventricular septal defects (PM VSDs) usually present early with heart failure and pulmonary hypertension. Rarely, delayed presentation occurs due to spontaneous partial closure by adjacent structures, most commonly the septal tricuspid leaflet (STL). We report a rare case of a 13‐year‐old boy with a large PM VSD partially closed by the anterior tricuspid leaflet (ATL), associated with a hypoplastic STL. The patient underwent successful surgical VSD closure with tricuspid valve repair. This unusual mechanism of partial spontaneous closure highlights the anatomical variations that influence the natural history of PM VSDs and emphasizes the importance of detailed echocardiographic evaluation.
