Key result
Lead indwell times under 12 years linked to ~5-fold higher transvenous lead extraction success.
Why the study?
Lead extraction is increasingly needed, and while lead indwell time is known to influence procedural success, the factors affecting complete radiological success required further examination.
Do lead properties such as indwell time and ICD lead presence affect complete radiological success in transvenous lead extraction?
Observational (n=102)
No
Do lead properties such as indwell time and ICD lead presence affect complete radiological success in transvenous lead extraction?
Lead indwell time exceeding 12 years and the presence of ICD leads significantly reduce the likelihood of complete radiological success during transvenous lead extraction, necessitating more complex extraction techniques.
May inform extraction planning by dwell time; hypothesis-generating for complex techniques in long-dwelling ICD leads.
Background The insertion of cardiac implantable electronic devices has become increasingly common, resulting in a growing need for lead extraction, most often due to infection and malfunction. The majority of the research indicated that the lead indwell time was the most important factor in determining the procedural success of extraction. In this study, we examined the factors affecting complete radiological success. Methods A total of 192 lead extractions were performed on 102 patients at Başakşehir Çam and Sakura City Hospital. These procedures were compared based on lead indwell time and lead properties, considering the complete radiological success. Results Complete radiological success was determined by lead indwell time and the presence of ICD leads. For leads with an indwell time of less than 12 years, the complete radiological success increased more than fivefold. Furthermore, in patients with ICD leads and lead indwell time exceeding 12 years, more complex and additional extraction techniques may be beneficial rather than simple traction alone. Conclusion Prolonged lead indwell time and the presence of ICD leads were identified as two key factors affecting both complete radiological success and procedural difficulty.

Erdem et al. (2026) conducted an observational in Patients undergoing transvenous lead extraction (n=102). Lead indwell time and lead properties (ICD leads) vs. Shorter indwell time and non-ICD leads was evaluated on Complete radiological success. Lead indwell time of less than 12 years increased complete radiological success of transvenous lead extraction more than fivefold compared to longer indwell times.
