Objective Perioperative transesophageal echocardiography (TEE) can be a vital adjunct to guide precise intervention during resection of a giant right ventricular cardiac schwannoma. Case Presentation A 30-year-old woman presented to our unit for resection of a giant right ventricular cardiac schwannoma of the base of the heart. Intervention A successful on-pump beating heart resection of the 1.3-kilogram cardiac schwannoma was performed. Intraoperative TEE with 3-dimensional multiplanar reconstruction helped to identify the thin capsule predominantly along the inferior right ventricular wall; monitor for ischemia during tumor dissection and coronary vessel clamping; monitor for intracardiac air in the left heart during beating heart excision; and post-intervention verification of complete tumor excision, preservation of biventricular function, and integrity of mitral and tricuspid valves. Conclusion Real-time imaging with perioperative transesophageal echocardiography is a vital current adjunct to multimodality imaging in transforming complex cardiac tumor surgery into a precise and successful intervention with good outcomes.
