Abstract Clear-cell renal cell carcinoma (ccRCC) is the most common histological subtype among renal cell carcinomas. Recently, significant advances in its treatment have allowed for a relevant increase in patient survival. In this regard, imaging methods for precise assessment of this disease are progressively gaining more importance. Here, we report a case of a patient with ccRCC with bone-exclusive metastases in whom positron emission tomography/computed tomography (PET/CT) with prostate-specific membrane antigen (PSMA) was used for staging and assessing treatment response. The exam showed superior performance compared with conventional imaging studies, including PET/CT with fluorodeoxyglucose (FDG). It enabled us to correctly diagnose the patient in addition to identifying a greater number of secondary metastases. Recent studies have evaluated the role of PSMA PET/CT in ccRCC, with promising results. The present case report aligns with these findings to corroborate the superior performance of PSMA PET/CT in assessing ccRCC compared with conventional imaging studies.
