Key result
Transjugular TEER with PASCAL successfully reduces severe MR to mild.
Why the study?
Mitral transcatheter edge-to-edge repair is a reasonable strategy in severely symptomatic patients with primary severe mitral regurgitation at prohibitive surgical risk.
Does right internal jugular mTEER with the PASCAL system allow successful treatment of severe primary mitral regurgitation in a patient with complex anatomy preventing transfemoral access?

Does right internal jugular mTEER with the PASCAL system allow successful treatment of severe primary mitral regurgitation in a patient with complex anatomy preventing transfemoral access?
Transjugular venous access with the PASCAL system provides a feasible and effective alternative for mitral transcatheter edge-to-edge repair in patients with severe mitral regurgitation whose complex anatomy precludes standard transfemoral access.
Transjugular PASCAL TEER may be feasible when femoral access is precluded; leaves open safety and durability questions for prospective evaluation.
In severely symptomatic patients with primary severe mitral regurgitation at high or prohibitive surgical risk, mitral transcatheter edge-to-edge repair is a reasonable strategy. We present a successful case of right internal jugular vein access using PASCAL mitral valve repair (Edwards Lifesciences) in a patient with prohibitive risk for surgical mitral repair.

Hu et al. (2024) conducted a case report in Severe primary mitral regurgitation (n=1). PASCAL mitral valve repair system via right internal jugular vein was evaluated on Procedural success and reduction of mitral regurgitation. Transjugular transcatheter edge-to-edge repair utilizing the PASCAL system was successfully performed, reducing severe mitral regurgitation to mild with significant symptomatic improvement.
