Abstract Background Despite high symptom burden, few patients with heart failure (HF) are referred to palliative care (PC), and often late in the disease trajectory. The Horizon Europe RAPHAEL project aims to address this by integrating a palliative care approach, including remote symptom monitoring, into HF management. As part of this work, an online training program was developed for healthcare professionals. Purpose To describe the structure and development of the RAPHAEL training program designed to enhance professionals’ competence in delivering multidisciplinary palliative care for patients with HF. Methods The training was developed collaboratively within the RAPHAEL consortium. Learning objectives were drafted and refined, an eight-module structure was established, and consortium experts contributed lectures. A prototype module was reviewed for technical and pedagogical quality and used as a template for subsequent modules. Relevant tools and reference materials were curated, and a digital platform and assessment format were selected. Results The final program comprises eight modules and 22 expert lectures covering: HF; comorbidities and needs assessment; advance care planning; adaptation of medical treatment; informal caregiving; inclusive communication; social, spiritual, existential, and ethical concerns; and symptom monitoring with PAL@HEART. Each module includes a pre-learning self-assessment, recorded lectures, and supplementary materials. The program concludes with an assessment requiring use of the PAL@HEART app with one patient and a reflective report. Conclusion Using multidisciplinary expertise within a European consortium proved effective for developing a high-quality training program. The RAPHAEL program supports improved competence in palliative HF care and may serve as a model for integrating palliative care into chronic disease management across Europe.
