IVT can be started earlier than MT, and in some patients, IV tPA might completely lyse proximal occlusions before MT starts. A recent meta-analysis found a complete recanalization within 3 hours after application of IV tPA in 21% of M1 occlusions, in 38% of M2 occlusions, and in 4% of internal carotid artery (ICA) occlusions.
