Abstract Introduction: Effective management of patients with diabetes relies heavily on patients’ adherence to prescribed medication. However, a validated Bengali version of the Hill-Bone Medication Adherence Scale suitable for measuring adherence in hospital settings was lacking. The objectives of this study were to assess the reliability and construct validity of the Bengali-translated scale among diabetes patients. Materials and Methods: This cross-sectional study was conducted among 180 Bengali-speaking diabetes patients on antidiabetic medication attending the medicine outpatient department at a tertiary care hospital in Eastern India with interviewer administered questionnaire. The manuscript was prepared in accordance with the Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) guidelines. Results: Mean age of the participants was 57.6 ± 10.8 years. The scale demonstrated strong internal consistency (Cronbach’s α = 0.84). Exploratory factor analysis revealed a one-factor model comprising eight items, explaining 40.3% of the variance. CFA supported the model with acceptable fit indices (Comparative Fit Index = 0.93, Tucker–Lewis Index = 0.91, standardised root mean square residual = 0.04). Conclusions: 8-items Bengali version of the HB-MAS proved to be a reliable and valid instrument for evaluating medication adherence among Bengali-speaking diabetes patients in clinical settings.
