Catheter-directed tPA reduces native aortic root thrombus in a pediatric Fontan patient.
Case Report (n=1)
Highlights the importance of echocardiographic evaluation and monitoring for life-threatening native aortic root thrombus in patients with hypoplastic left heart syndrome.
Catheter-directed tPA may reduce aortic root thrombus in rare pediatric HLHS Fontan cases; single report leaves open safety, efficacy, and generalizability.
•Native aortic root thrombus is a life-threatening complication in patients with HLHS.•Echo evaluation of the native aortic root is an important consideration.•Echo monitoring is a useful tool for assessment of thrombus response to therapy.

Segar et al. (2021) conducted a case report in Native aortic root thrombus in hypoplastic left heart syndrome (n=1). Alteplase (tPA) was evaluated. Catheter-directed tPA therapy effectively reduced the native aortic root thrombotic burden in a 3-year-old HLHS patient post-Fontan, though the patient ultimately died from subsequent complications.
