Why the study?
Cardiac implantable electronic device infections range widely in severity, and pocket infection complications and systemic signs critically impact prognosis and treatment.

This case highlights the challenges in managing complicated CIED pocket infections in patients with high comorbidities, sometimes necessitating epicardial pacemaker implantation.
Complicated CIED infection after battery replacement in comorbid patients warrants close monitoring; leaves open optimal prevention and management strategies.
Cardiac implantable electronic device (CIED) infections can present with a large spectrum ranging from erythema due to skin site inflammation to infective endocarditis, which can be fatal. Whether the pocket infection is complicated or not and whether there are signs of systemic infection are important in the prognosis and treatment of patients. In this case report, we present a patient with high comorbidity who underwent pacemaker battery replacement and developed complicated pocket infection and the difficulties in management.
