Key result
The combination of high brachial-ankle pulse wave velocity (≥1,531 cm/s) and high atherogenic index of plasma (≥0.35) significantly predicted major adverse cardiovascular events (HR 3.38) compared to lower values.
Why the study?
Combining different methods can improve cardiovascular risk prediction, but whether combining atherogenic index of plasma and brachial-ankle pulse wave velocity adds prognostic value for MACE was unknown.
Does combining the atherogenic index of plasma (AIP) and brachial-ankle pulse wave velocity (baPWV) improve the prediction of major adverse cardiovascular events in individuals without cardiovascular disease?

Combining the atherogenic index of plasma and brachial-ankle pulse wave velocity significantly improves cardiovascular risk stratification and MACE prediction in individuals without known cardiovascular disease.
Background and Objectives: Combining different methods can improve the ability to predict cardiovascular risk. Here, we aimed to determine whether combining the atherogenic index of plasma (AIP) and brachial-ankle pulse wave velocity (baPWV) has additional prognostic value for predicting major adverse cardiovascular events (MACE).Methods: We retrospectively analyzed 7,409 consecutive individuals without documented cardiovascular disease and with data about blood cholesterol and baPWV (mean age, 60.311.9years; female, n=3,297; 44.5%).We calculated AIP as log (triglyceride/high-density lipoprotein cholesterol) and assessed death, non-fatal myocardial infarction, coronary revascularization, and ischemic stroke. Results: During a median followup of 3.98 (interquartile range, 1.80-5.88)years, 152 (2.1%) participants developed MACE.The results of multivariable analyses revealed that baPWV + AIP could predict MACE more effectively when combined, compared with either method alone (baPWV and AIP: <1,531 cm/s and <0.35 vs. baPWV 1,531 cm/s and 0.35, respectively: hazard ratio, 3.38; 95% confidence interval, 1.95-5.86;p<0.001).Global chi-square values significantly increased from 72 to 102 (p<0.001) when combined AIP + baPWV data were added to clinical data. Conclusions: Combined baPWV + AIP data increased the ability to predict MACE in persons without cardiovascular disease.Considering the simplicity of measuring baPWV and AIP, such data might be useful for cardiovascular risk stratification.

Kim et al. (2024) conducted a cohort in Without documented cardiovascular disease (n=7,409). High brachial-ankle pulse wave velocity (≥1,531 cm/s) and high atherogenic index of plasma (≥0.35) vs. Low baPWV (<1,531 cm/s) and low AIP (<0.35) was evaluated on Major adverse cardiovascular events (MACE) (HR 3.38, 95% CI 1.95-5.86, p=<0.001). The combination of high brachial-ankle pulse wave velocity (≥1,531 cm/s) and high atherogenic index of plasma (≥0.35) significantly predicted major adverse cardiovascular events (HR 3.38) compared to lower values.
