Purpose: Whereas previous studies focused on monocular contrast sensitivity function (CSF), the binocular CSF in amblyopic children has not been fully investigated. In this study, we aimed to investigate binocular CSF in children with anisometropic amblyopia (AA), focusing on its presence regardless of successful treatment, and to explore the contributing factors. Methods: Twenty untreated children with AA (median age = 8.5 years), 20 treated children with AA (TA; 7.5 years), and 15 age-matched normal controls (NCs; 8.0 years) underwent visual acuity (VA) assessment, refraction, TNO stereopsis, and monocular/binocular CSF testing using the quick CSF method. Binocular summation ratio (BSR) was calculated as binocular CSF divided by the better-eye CSF. Results: Amblyopic eyes in the AA group showed significant reductions in CSF compared to NCs (P = 0.022). Binocular CSF, however, did not differ between groups (all P > 0.05). The AA group exhibited reduced BSR (P = 0.039). Binocular area under the log under CSF (AULCSF) correlated strongly the contrast sensitivity (CS) at 6 cycles per degree (cpd; r = 0.852, P < 0.0001) and AULCSF in the fellow eye (r = 0.862, P < 0.0001). Conclusions: Despite monocular CSF deficits and reduced BSR in children with AA, binocular CSF remains comparable to that of NCs, suggesting compensatory mechanisms in binocular summation.
