心房細動(Atrial fibrillation:AF)は治療を必 要とする不整脈の中でもっとも頻度が高く,生 命予後,機能予後を有意に悪化させるため,決 して放置してはならない. 多数の臨床研究 (104 報の合計 9,686,513 例)のメタ解析結果では 1) , 心房細動を有する 587,867 例(6.1%)は有さな い例に比し,心不全の相対リスクが 4.99 倍,脳 卒中は 2.42 倍,総死亡が 1.46 倍,心臓突然死が 1.88 倍増加していた. AFの有病率は年齢とともに増加することが 知られているが 2) ,最近の山形県の検診データ (2017 年)では,80 歳以上の男性のAF有病率は 7.4%であった 3) .検診では持続性(慢性)AFの 診断が中心であり,高齢者にも頻度が少なくな い発作性AFを加えると 4) ,80 歳以上の男性のAF 有病率は10%を超える可能性がある. つまりAF は高齢者のcommon diseaseと言える.重要なこ ととして,AF例では高齢になるほど血栓塞栓症 のリスクが高くなり 5) ,さらに生命予後を悪化 させる心不全合併例も増加することがある 6) . わが国の平均寿命は男女ともに世界でトップク ラスにあるが,健康寿命は男性で約 9 年,女性 で約 12 年短い 7) .健康寿命短縮の大きな要因は 認知症と脳血管障害であり 8) ,とくにAFに起因 する心原性脳梗塞(脳塞栓症) の重症度は高 く 9) ,AFの適切な管理治療が健康寿命延伸のた めにも必須となる.本稿では,AFの最新の治療 について,日本人高齢AF患者に対する治療に関 する新しい知見, エビデンスを中心に述べたい. 1.AFに対する包括的管理・治療指針 2020 年ヨーロッパ心臓病学会(ESC)の心房 細動治療ガイドラインは 10) ,シンプルかつ包括 的なAF治療の進め方として,ABCパスウェイを 提唱,推奨している.'A'はAnticoagulation/Avoid stroke(抗凝固療法と脳卒中予防) ,'B'はBetter symptom management(症状管理) ,'C'はCardio-済生会熊本病院循環器内科 119 th Scientific Meeting of the Japanese Society of Internal Medicine:Invited Lecture:1.Comprehensive treatment of atrial fibrillation in elderly and very elderly patients.
