Key result

The combination of cisplatin and etoposide demonstrated minimal activity in advanced and metastatic adrenocortical carcinoma, yielding an objective response rate of 11% (95% CI, 3.7-24%).

Implication

Minimal activity of etoposide-cisplatin in metastatic adrenocortical carcinoma; leaves open whether other regimens merit prospective evaluation.

Structured PICO

Does the combination of cisplatin and etoposide followed by mitotane improve objective response rates in patients with advanced or metastatic adrenocortical carcinoma?

P
Population
45 eligible patients with advanced, unresectable, or metastatic adrenocortical carcinoma with objectively measurable disease.
I
Intervention
Cisplatin 50 mg/m2 IV on Days 1 and 2, and etoposide 100 mg/m2 on Days 1, 2, and 3, repeated every 21 days. At disease progression, patients without prior mitotane received mitotane 1000 mg orally 4 times a day with cortisone acetate and fludrocortisone acetate.
O
Outcome
Objective response ratesurrogate

The combination of cisplatin and etoposide has minimal activity in advanced and metastatic adrenocortical carcinoma, indicating that other treatment strategies are warranted.

Williamson et al. (2000) studied locally advanced or metastatic adrenocortical carcinoma (n=45). Cisplatin and etoposide followed by mitotane was evaluated on Objective response rate (95% CI 3.7-24). The combination of cisplatin and etoposide demonstrated minimal activity in advanced and metastatic adrenocortical carcinoma, yielding an objective response rate of 11% (95% CI, 3.7-24%).
