Case Report (n=1)
Ultra-high-density electroanatomic mapping can successfully identify critical atrio-atrial connections as ablation targets for complex atrial tachyarrhythmias following biatrial orthotopic heart transplantation.
Supports ultra-high-density mapping to target post-transplant atrio-atrial connections; leaves open confirmation in larger prospective cohorts.
Orthotopic heart transplantation (OHT), using either the bicaval or biatrial anastomosis technique, is the golden standard for end-stage heart failure. Biatrial OHT involves anastomosing the donor and recipient atrial cuffs. A relatively common complication of biatrial OHT is atrial tachyarrhythmia (AT), which may include complex, scar related micro- or macro-reentrant tachycardias.1

Teunissen et al. (2025) conducted a case report in Atrial tachyarrhythmia after biatrial orthotopic heart transplantation (n=1). Ultra-high-density mapping and radiofrequency ablation was evaluated on Termination of atrial tachyarrhythmia and long-term recurrence.
