Ultra-high-density mapping identifies and guides successful ablation of a tiny microreentrant atrial tachycardia circuit.
Case Report (n=1)
Ultra-high-density mapping can definitively identify and characterize microreentrant tachycardia circuits at prior ablation gaps, distinguishing them from focal tachycardias and guiding successful targeted ablation.
May aid targeted ablation of microreentrant circuits post-AF ablation; leaves open generalizability beyond this single case.
KEY TEACHING POINTS•A proportion of atrial tachycardias post–atrial fibrillation ablation have a microreentrant mechanism.•These microreentrant circuits usually occur at sites of gaps in ablation lines.•High-resolution mapping systems allow small reentrant circuits to be defined and characterized and aid the targeting of ablation therapy for microreentrant tachycardias. Ng et al. (2017) conducted a case report in Recurrent atrial fibrillation and atrial tachycardia post-ablation (n=1). Ultra-high-density electroanatomic mapping (Rhythmia system) and ablation was evaluated on Successful mapping and termination of tachycardia circuit. The ultra-high-density mapping system successfully identified and mapped a 0.32 cm2 microreentrant left atrial tachycardia circuit at an ablation gap, allowing for targeted ablation and termination.
