The purpose of the present investigation was to study the possibility of improving the hearing aid treatment of patients suffering from high tone loss with recruitment. Unlike previous projects of similar character where the aim was to obtain maximum speech intelligibility, our aim was to determine the frequency response curve giving the most comfortable sound impression for the patient. With this in mind our hypothesis is that the optimal frequency response will depend upon the input sound pressure. It is postulated that the frequency response should be flat up to a given frequency, the audiogram's cut off frequency, and thereafter should rise with a variable slope to a certain level (treble amplification), slope and treble amplification being functions of input sound pressure level. Ten patients were examined with response curves synthesized on laboratory equipment using earphone presentation. Current speech was used as test signal. In the first part of the examination the test promotor chose at random the test apparatus parameters input sound pressure level and slope, whilst the test person adjusted the treble amplification. In the second part of the examination the test promotor chose the input sound pressure level and the treble amplification whilst the person adjusted the slope. An analysis of the results strongly indicate a preference towards a lower slope with increasing sound pressure. With maintained slope it was found that a 10 dB increase of sound pressure level on an average resulted in a reduction of approximately 6 dB of desired treble amplification. On these results, the hypothesis that a reduction in the treble amplification is preferred with increasing sound pressure input level, can not be rejected.FormÅlet med denne undersøgelse har været at studere muligheden for forbedring af høreapparatbehandlingen af patienter, der lider af high tone loss med recruitment. Til forskel fra tidligere undersøgelser af lignende karakter, hvor formÅlet var at opnÅ maksimal taleforstÅelighed, har man ved denne undersøgelse søgt at bestemme den frekvenskarakteristik, som giver patienten det mest behagelige lydindtryk. Med dette som grundlag er vor hypotese, at den optimale frekvenskurve vil være afhængig af indgangslydtrykket. Det postuleres, at frekvensgangen bør være flad op til en given frekvens, audiogrammets knækfrekvens, for derefter at være variabel i hældning op til et vist niveau (diskantforstærkning), idet hældning og diskantforstærkning er funktioner af indgangslydtrykket. Tl patienter er blevet undersøgt med frekvenskurver, syntetiseret ved hjælp af en laboratorieopstilling, med lydpræsentation via hovedtelefoner. Løbende tale blev brugt som testsignal. 1 den første del af undersøgelsen valgte forsøgslederen vilkÅrligt testapparaturets parametre, indgangslydtryk og hældning, medens forsøgspersonen indstillede diskantforstærkningen. I undersøgelsens anden del valgte forsøgslederen indgangslydtryk og diskantforstærkning, medens forsøgspersonen indstillede hældningen. Ved analyse af resultaterne viser der sig et stigende antal ønsker om mindre hældning ved stigende indgangslydtryk. Ved fastholdt hældning fandt man, at en 10 dB's forøgelse af indganglydtrykket i gennemsnit resulterer i en reduktion af den ønskede diskantforstærkning pÅ omkring 6 dB. Udfra disse resultater kan hypotesen, der udtrykker, at ved stigende indgangslydtryk ønskes faldende diskantforstæerkning, ikke forkastes.
