Why the study?
Mechanical circulatory support with ECMO and the recently introduced Impella is essential for severe heart failure and cardiogenic shock refractory to medical therapy, necessitating an overview of their perioperative management.
This educational review provides a comprehensive overview of the physiological principles, indications, and perioperative management of ECMO and Impella in patients with cardiogenic shock.
IMPELLA availability since 2017 broadens acute HF support options in Japan; leaves open optimal ECMO-IMPELLA combination strategies.
しかし急性の心不全・心原性ショックに対する初期治療としてのIABP(intraaortic balloon pumping),ECMO(extracorporeal membrane oxygenation, もしくはpercutaneous cardiopulmonary support[PCPS])の重要性は変わっておらず,今後も変わらないと思われる.2017年より,わが国においても循環補助用心内留置型ポンプカテーテルのIMPELLAの臨床使用が可能となり,これにより急性心不全に対する補助循環治療戦略もさらなる広がりをみせている.ECMOおよびIMPELLAの周術期管理について概説する.
