Increasing afterload and reducing IABP support resolves dynamic LVOTO and SAM after Cone reconstruction.
Why the study?
Dynamic left ventricular outflow tract obstruction with systolic anterior motion during Cone operation for adult Ebstein's anomaly is a rare and challenging complication requiring detailed management strategies.
Case Report (n=1)
No
Dynamic LVOTO with SAM can occur during the Cone operation for Ebstein's anomaly in adults and may be paradoxically worsened by IABP, necessitating afterload augmentation with vasopressors to restore hemodynamics.

Matsuba et al. (2026) conducted a case report in Ebstein's anomaly with dynamic left ventricular outflow tract obstruction and systolic anterior motion (n=1). Cone operation with afterload augmentation (noradrenaline and vasopressin) and reduced IABP support was evaluated on Resolution of systolic anterior motion and dynamic left ventricular outflow tract obstruction. Augmenting afterload with noradrenaline and vasopressin while reducing IABP support successfully resolved dynamic LVOTO and SAM following a Cone operation for Ebstein's anomaly.
