Key Points

To evaluate the efficacy of small-volume hypotensive resuscitation using 7.5% NaCl supplemented with adenosine, lidocaine, and magnesium in a porcine model of severe hemorrhagic shock.
Female pigs (35–40 kg, N=16) underwent controlled bleeding to a mean arterial pressure of 35–40 mm Hg for 90 minutes in a laboratory setting.
Animals were randomly assigned to receive an intravenous bolus of 4 mL/kg 7.5% NaCl plus adenosine, lidocaine, and magnesium (n=8) or 4 mL/kg 7.5% NaCl alone (n=8) during 60 minutes of hypotensive resuscitation, followed by shed blood return.
At 60 minutes of hypotensive resuscitation, the intervention group had significantly higher mean arterial pressure (48 mm Hg [95% CI, 44–52] vs 33 mm Hg [95% CI, 30–36], p < 0.0001), cardiac index (76 mL/min/kg [95% CI, 63–91] vs 47 mL/min/kg [95% CI, 39–57], p = 0.002), and oxygen delivery (7.6 mL O2/min/kg [95% CI, 6.4–9.0] vs 5.2 mL O2/min/kg [95% CI, 4.4–6.2], p = 0.003).
Treated pigs had significantly lower blood lactate (7.1 mM [95% CI, 5.7–8.9] vs 11.3 mM [95% CI, 9.0–14.1], p = 0.004) and higher base excess (-5.9 mEq/L [95% CI, -8.0 to -3.8] vs -11.2 mEq/L [95% CI, -13.4 to -9.1]).
At 180 minutes, treated animals maintained a three-fold higher urinary output (2.1 mL/kg/hr [95% CI, 1.2–3.8] vs 0.7 mL/kg/hr [95% CI, 0.4–1.2], p = 0.001) and lower plasma creatinine levels.
