The case is reported of an elderly man who presented with pancytopenia and massive splenomegaly. Intensive investigation failed to reveal the ætiology. Splenectomy was performed, after which there was a dramatic improvement clinically and hæmatologically. Histological examination of the spleen revealed only non-specific changes. The clinical, hæmatological and histological findings in this man are classical of non-tropical idiopathic splenomegaly, except for the presence of a moderately elevated IgM level.
