A 13-year-old girl underwent abdominal ultrasound because of elevated liver enzymes and an intrahepatic mass was found. Subsequent MRI revealed a large liver tumor with a central scar and enlarged lymph nodes in the upper abdomen. F-18 FDG PET was performed for whole body staging. Avid tracer uptake was noted in the primary tumor and the suspicious lymph nodes, thus confirming the MRI findings. The patient underwent hemihepatectomy and the tumor was histopathologically identified as fibrolamellar carcinoma, a rare and distinctive variant of hepatocellular carcinoma that was first described in 1956.
