Abscess formation in the intervalvular fibrosa with dual fistulous tracts communicating with the aortic root and ascending aorta is a rare complication of infective endocarditis sparsely described in the literature.
This case highlights the critical importance of multimodal imaging, particularly combining echocardiography with CT coronary angiography, for the accurate diagnosis of complex and life-threatening complications of infective endocarditis such as MAIVF pseudoaneurysms and aorto-cavitary fistulae, enabling timely life-saving surgical intervention.
