Why the study?
Patients with T2DM frequently have coexisting hypertension and high cardio-renal risk, but monotherapy-centric or stepwise approaches may leave residual cardio-renal-metabolic risk.
What are clinician expectations regarding the use of telmisartan + dapagliflozin combination therapy for cardio-renal-metabolic risk management?
What are clinician expectations regarding the use of telmisartan + dapagliflozin combination therapy for cardio-renal-metabolic risk management?
Clinicians strongly support integrated cardio-renal-metabolic management and view the telmisartan-dapagliflozin combination as a pragmatic option for complex cardiometabolic patients.
Clinician expectations for Telmi in T2DM-hypertension map potential use; leaves open real-world CRM impact pending prospective data.
Background: Patients with type 2 diabetes mellitus (T2DM) frequently have coexisting hypertension and are at increased risk of cardiovascular (CV) and renal complications. Monotherapy-centric or stepwise treatment approaches may leave substantial residual cardio-renal-metabolic (CRM) risk, underscoring the need for integrated therapeutic strategies. Objective: The objective is to assess clinician expectations regarding patient selection, anticipated benefits, and intended use of a Telmisartan + Dapagliflozin combination, including its potential role as a fixed-dose combination (FDC), in routine cardiometabolic care. Materials and Methods: SYNERGY Insights was a cross-sectional, questionnaire-based survey conducted among a large, multidisciplinary cohort of clinicians. The survey evaluated perceptions of residual risk with monotherapy, importance of integrated CRM management, relevance of the Telmisartan + Dapagliflozin combination, anticipated patient profiles, primary clinical objectives, prescribing volume, and preferred dosing strategies. Data were analyzed descriptively. Results: A total of 2775 clinicians participated. Most respondents reported a high coexistence of T2DM and hypertension and perceived monotherapy-centric approaches to leave moderate-to-strong residual cardio-renal risk. Integrated therapy was rated as important or extremely important by the majority. The Telmisartan + Dapagliflozin combination was considered clinically relevant, particularly for patients with T2DM and hypertension, high CV risk, and early diabetic kidney disease. Most clinicians have opined on the benefits exerted by the FDC, such as integrated glycemic and blood pressure control, CV risk reduction, and renal protection. Conclusion: The findings demonstrate strong clinician support for integrated CRM management and position the Telmisartan + Dapagliflozin combination as a pragmatic option for managing complex cardiometabolic patients in routine practice.
