Adult-onset Still disease (AOSD) is a rare systemic inflammatory disorder marked by quotidian fever, arthralgia/arthritis, and an evanescent salmon-colored rash. Pulmonary involvement is uncommon but may include mild symptoms or, rarely, acute respiratory distress syndrome (ARDS)—a severe, potentially life-threatening complication. ARDS may occur at disease onset or during flares despite adequate treatment. We report 3 cases of AOSD flares presenting with ARDS, detailing their clinical, biological, and radiologic features, as well as the therapeutic approaches employed.
