Overview

May warrant high intervention in obstetric ICU cases despite low APACHE scores; case reports leave optimal protocols open for prospective study.

Structured PICO

P
Population
46 pregnant patients admitted to an 8-bed general intensive care unit at a tertiary care university-affiliated hospital over a 4-year period. Group 1 (n=19) requiring mechanical ventilatory support, Group 2 (n=27) requiring intensive monitoring. Commonest admission diagnoses were pregnancy-induced hypertension and hemorrhage.
I
Intervention
Admission to general intensive care unit for mechanical ventilation or intensive monitoring
O
Outcome
Course, interventions required to achieve predetermined end-points (systolic blood pressure 110-150 mmHg, urine output >= 1 cc/kg/h, oxygen saturation > 95%), and outcome (mortality, length of stay)

Obstetric patients admitted to a general ICU for ventilation or monitoring require a high level of intervention despite low APACHE scores and short lengths of stay.
