Why the study?
Randomised controlled trials showed that early survival gains of EVAR over open repair tend to vanish after four years, prompting a review of techniques and strategies.
Does endovascular repair improve survival compared to open repair in patients with abdominal aortic aneurysms?
Does endovascular repair improve survival compared to open repair in patients with abdominal aortic aneurysms?
While EVAR offers early survival benefits over open repair for AAA, these advantages appear to diminish after four years of follow-up.
Early EVAR survival gains over open AAA repair fade after 4 years; leaves open optimal long-term strategy and patient selection.
Open and endovascular repair of abdominal aortic aneurysms (AAA) are being extensively practised today. Comparison of both endovascular (EVAR) and open repair (OSR) in randomised controlled trials confirmed that early gains in survival and aneurysm-related mortality tend to vanish after four years of follow-up. The aim of this paper is to review open and endovascular repair techniques, their early and late benefits, based on scientific evidence and experience from an academic vascular unit, and to assess present and future strategy for management of both intact and ruptured AAA.
