May support lower BP thresholds for HTN diagnosis in women; leaves open whether SCORE modification improves risk stratification.
Background: SCORE scale is widely used for the estimation of 10-year risk of fatal cardiovascular disease (CVD). In those with low risk levels intensive risk modification is particularly important. For this purpose SCORE charts are available for high-density lipoprotein cholesterol (HDL-C). The risk level in women is always lower than in men with similar characteristics. There are few recommendations for the lower blood pressure (BP) thresholds for the diagnostics of hypertension (HTN). Aim: To compare the relative risk of fatal CVD in men and women with untreated essential HTN without established CVD and diabetes mellitus with real outcomes according to prospective evaluation. Methods: We included 385 patients (131 men and 254 women) with untreated HTN (mean age 51.4±7.6y). For all patients initial risk level of fatal CVD was estimated on inclusion. The data about outcomes after 8 years follow-up (mean 8.0±1.1y) were obtained in 356 patients (92.7%). All patients were on regular antihypertensive and hypolipidemic treatment within the follow-up period. Results were processed with Statistica 8.0 software.
