Supports caution with sotalol in ischemia; leaves open translation to human proarrhythmic risk and SWORD mortality.
In an in vitro guinea-pig model, DL- and D-sotalol exhibited proarrhythmic effects during ischemia by increasing spontaneous arrhythmias, providing a potential mechanistic explanation for the increased mortality observed in the SWORD trial.
